Job Description:
Exciting opportunity to join a team of highly motivated and
energized professionals. Must be able to function at a high level
with little oversight on a Monday - Friday day shift schedule,
coordinate and provide nursing care, training, education and home
support needed for patients. Must work collaboratively and develop
relationships with team members, new staff and skilled nursing home
facility representatives; collect and record relevant patient
health data, develop patient care plans and handle EMR
documentation.BS in Nursing or AA/diploma from Nursing School with
at least 3 months recent hemodialysis experience needed. Need valid
RN license, BLS certification, reliable daily transportation, able
to travel to local area sites and to lift 50 pounds.
