Job Description:
* RN Field Case Managers are responsible for comprehensive
assessments in the patients home, including the completion of OASIS
using electronic documentation.* In addition to OASIS assessments,
RN Field Case Managers perform skilled nursing visits as needed and
are responsible for delegation and coordination of care with LVN
and HHA team members* Must be fully skilled in lab draws, catheter
placement, wound care , wound vac and IV*Experience: *Certified RN
required (BLS/CPR)Job Type: Full-timePay: $45.00 - $80.00 per
