to learn! Job Summary and Qualifications Registered Nurse - Adult
Congenital Heart Disease (ACHD) Program Coordinator Reports To:
Practice Manager Location: Medical City Dallas General Summary of
Duties: The ACHD Program Coordinator is responsible for the ongoing
development, coordination, and management of the Adult Congenital
Heart Disease (ACHD) service line. This role ensures high-quality
clinical care throughout the patient experience and supports the
operational and strategic goals of the program. The coordinator
works collaboratively with medical, administrative, and
multidisciplinary teams to implement program initiatives, improve
patient outcomes, and manage new referrals, assist with
coordination of care. ________________________________________ Key
Responsibilities: • Serve as the primary liaison for the ACHD
program, ensuring seamless coordination across departments as
requested by physician. • Manage all aspects of patient care from
initial contact through post-treatment follow-up, including
education, scheduling, and communication. • Lead and facilitate
multidisciplinary team meetings and ensure alignment on patient
care protocols. • Attend ACHD care conference • Collect, analyze,
and report on patient volumes, referrals, and program metrics
monthly. • Develop and maintain program policies, procedures, and
workflows to ensure compliance and efficiency. In conjunction w.
ACHD clinical team. • Provide training and guidance to office staff
on ACHD program protocols and patient management. • Coordinate and
support patient education initiatives, including postoperative
support groups. • Participate in special projects and clinical
initiatives as assigned by the Medical Director. • Ensure
compliance with all regulatory and organizational standards,
including the Physician Services Group Compliance Program and Code
of Business Practices. EDUCATION: • Minimum of Bachelor’s Degree in
Counseling OR in an accredited Nursing Program Required.
EXPERIENCE: • Previous experience in clinical nursing, cardiology
experience a plus. CERTIFICATION/LICENSE: • BSN Degree with Nursing
License in state of residency. KNOWLEDGE, SKILLS AND ABILITIES: •
Knowledge of organizational policies, procedures and systems. •
Knowledge of computer systems and applications. • Knowledge of
regulatory guidelines and documentation requirements. • Ability to
maintain a positive attitude and work effectively under stress. •
Ability to take initiative and to exercise independent judgment,
decision-making and problem- solving expertise. • Interpersonal
skills to successfully interact with practice leadership and staff
