Job Description:
AccentCare is seeking a compassionate and dedicated RN
Registered Nurse to join our Hospice team, providing end-of-life
care that respects the dignity and comfort of our patients. In this
role, you will work closely with patients and their families to
deliver personalized care based on individual needs, reflecting our
commitment to enhancing the well-being of our clients. This
position requires a keen sensitivity to the emotional and physical
needs of patients in their final stages of life, as well as a
strong commitment to upholding the principles of palliative care.
Provide compassionate and comprehensive nursing care to hospice
patients. Assist in the development and implementation of care
plans tailored to individual patient needs. Educate and support
patients and their families about end-of-life care and comfort
measures. Collaborate with a multidisciplinary team to ensure
continuity of care. Administer medications and treatments as
prescribed by the attending physician. Monitor patient conditions
and report changes to healthcare providers. Ensure all care
practices comply with company policies and regulations. Strong
empathy and interpersonal skills Excellent communication and
documentation skills Ability to work independently and as part of a
team Proficient in managing symptoms Knowledge of healthcare
software and systems Medical Insurance Dental Insurance Vision
Insurance 401k Current RN license in the state of practice CPR
certification Certified Hospice and Palliative Nurse (CHPN)
