As an In Home Healthcare RN for Low Acuity patients, you will play a critical role in enhancing the lives of our patients by delivering high-quality care directly in their homes. This position demands a passionate individual who can assess patient conditions, administer prescribed medications, and collaborate closely with family members and a multidisciplinary team to ensure the most comfortable and effective care. This role requires adaptability to different home environments and a respectful understanding of patient privacy and dignity. Conduct initial patient assessments and develop individualized care plans. Administer medications and monitor patient recovery and progress. Educate patients and their families on treatment plans and in-home healthcare strategies. Maintain accurate patient records and report changes in patient condition to the appropriate supervisor or coordinator. Adhere to all industry standards and regulatory requirements for home healthcare. Strong interpersonal and communication skills Ability to work independently with minimal supervision Proficient in maintaining accurate healthcare documentation Capability to handle medical emergencies Flexibility to adapt to new environments and changes in patient conditions Medical Insurance Dental Insurance Vision Insurance Life Insurance Current Registered Nurse (RN) license in the state of employment Basic Life Support (BLS) Certification Valid driver’s license and reliable transportation
