the Position Job Title: Registered Nurse Profession: Nursing

Experience or 1 full year of Hospice Experience License: Must have
a valid nursing license in hand Certifications: CPR, BLS Must-Have:
Proficiency with HomeCare HomeBase EMR and OASIS within the last 6
months Reliable transportation, current driver’s license, and
agency-required liability insurance Description: Provides,
coordinates, and directs the provision of home nursing care
according to physician orders. Responsible for the competent
application of the nursing process based on agency policies and
procedures. Directly supervises and evaluates Home Health Aides and
Licensed Practical Nurses. Home visits required with periodic
office meetings. Must travel up to 75 miles for home visits.
Average number of patients per day ranges from 5-10. Contractor
will be assigned cases that equate to approximately 6-8 points
daily. Types of cases include Start of Care, Wound Care, and
Follow-up Visits. Expectation to accept all patients regardless of
distance from previous appointments. Documentation must be
completed within 24 hours of the appointment. No guaranteed hours;
productivity points of 30 must be achieved weekly. Overtime must be
pre-approved. Local candidates accepted at the same rate. Must be
comfortable with drives potentially longer than 20 minutes between
patients. Must have experience in home health. Requirements
